Appointment Scheduling:

 

Our Patient Care Coordinators will assist you in scheduling an appointment. 

​

• For Family Practices appointments, please call 714-352-5800.

• For Psychiatry office appointments, please call 714-543-5609.

Preparing For Appointment:

If you are a new patient, please arrive 15 minutes earlier than your scheduled appointment time. Our New Patient Packet is available below, please fill out the packet prior to your scheduled appointment. Please note, your insurance card and photo Identification will be requested at the time of check in.

​

Co-payment will be due at the time of check in.

​

Please remember to bring your Pharmacy information along with your list of medication and/or medication bottles.

​

If any change has occurred with your insurance or medical group please call our office prior to your scheduled appointment.
